Grant Application Forms
Requests for funding must be typed on the application form and submitted with the cover page. This form is available in Microsoft Word format and in Acrobat pdf format. The Word format will allow you to expand sections, however, you are encouraged to be brief. If additional space is needed, you may add extra pages.
Submissions must include twelve (12) copies of the entire application, including cover letter, application form, and all attachments, collated and stapled or clipped.
